Open the form
Tap the PDF in the bank's email. Tap the Share button (the square with the arrow) and choose eSign PDF.

Fill it in and sign
Tap each blank and type your answer. Add the date, tick the boxes, and draw your signature on the line. Zoom in with two fingers if the boxes are small.

Send it back
Tap Share and send it the way the bank asked: reply by email, or save it to Files and upload it on the bank's website.
If they can't see your signature
Your bank may open your file on a Windows computer. Some computers do not show a signature added with the iPhone's built-in tool. eSign PDF locks your answers and signature into the page (this is called flattening), so the bank sees what you see.
