eSign PDF / Guide
The email says "please sign and return." You're on your phone. Here's how to open it from your mail app, sign it, and get it back to them in the same thread.
Short answer
Open the PDF from the email. Use the Share button to send it to a signing app, and sign it. Then reply to the same email with the signed copy attached. Replying (not forwarding or starting a new email) keeps it in their thread, so they know what it is. Apple's free Markup works for a simple form. eSign PDF does it with no account for $59.99 one-time (US).

A reply you can copy
"Hi, I've attached the signed copy. Let me know if you need anything else." Short is fine. Keep the original file name so they can match it.
For a one-page form going to someone on an iPhone or Mac, Apple's Markup is enough. Tap the attachment, tap the pen, tap +, then Add Signature. Mail then offers to reply with it.
